  2. Ask your family doctor, surgeon, oncologist or other healthcare team member for a referral to a fertility specialist to learn more about your fertility options.

  3. Bring your fertility options exercise results, this decision aid, your notes, and any questions with you when you meet with your fertility specialist and oncologist.
